Scene 1
The history form asks for basic information like her
name, address, phone number, social security number, and birth date.
It also asks for her employer’s name and address, which she happens
to know because she fills out a lot of FedEx forms with the address,
even though she’s only been working there four months. She has
to pick an emergency contact, and thinks one that lives in Florida would
be a good idea, since her family is so many miles away. But she doesn’t
know who to put: not her boyfriend whom she’s only known for a
few months, not the Rabbi who is the friend of her family because she
doesn’t know his phone number by heart...she picks the one person
who has been her friend, although it seems so strained these days...but
she knows her friend will be more than willing to come to her rescue.
The only thing she can definitely rely on from that friend is that she’s
always willing to come fix all of her problems.
She fills in a lot of zeros with lines through them in the pregnancy section. They have to put the line at the law firm to designate what is a "zero" and what is the letter "O." It’s a force of habit. She racks her brain to fill in the details about her menstrual history, and comes up with something approximately right. Then she circles a lot of “NO” for symptoms she might be having "now" or "in the past." She tries to remember to read all the parts of the medical history, but it’s hard to concentrate since she hasn’t had any huge medical problems. No wisdom teeth and she’s allergic to penicillin, but that’s about it.
She hates the weight section. Her usual weight and her maximum weight represent a 40 lb difference and a world of struggle. But with that uncomfortable moment, she signs the form and turns it back to the receptionist.
